PATIENT FORMS
Do you feel frustrated when you arrive for a new appointment and are handed a bunch of forms to fill out? Me, too! Unfortunately, getting complete information from a new patient is essential to beginning a therapeutic relationship. So, here's one way to make things a little bit easier.

I will then send you a login and password for this website that will allow you to download all the intake forms you'll need prior to beginning your therapy. They're in PDF format. You can fill them out directly from your computer and e-mail them back to me, or you can print them and fill them out by hand and bring them with you on your first visit. As always, all information is kept strictly confidential. The forms all come directly to me.
